Directions for Completion and Submission of this Application
This application is to be completed whenever an ACHENA-accredited school or program implements a substantive change. See the Substantive Change Policy in Part III of the ACHENA Policies and Procedures Manual for examples of substantive changes and information on the substantive change application process. If it is unclear whether a change is considered substantive, contact accreditation@achena.org

This application must be submitted 90 days prior to projected implementation and must be approved by ACHENA for the implementation of the substantive change to occur.
